kzphoto on "Wordpress set-up error"

I've followed the set-up instruction and uploaded all the resources to my ftp server. When I going to my site, the set-up screen is not appear, it just show me a sample blog.

When I trying to login from the site, it's error.

What can I do? or anything I was doing wrong?
